IMPACT LIFE GROUP HEALTHCARE BROKER COMPANY

Impact Life Group, a healthcare broker company, believes in enabling the patient to become more active in their healthcare. We believe in "Patient Activation", which means providing the patient/health care consumer with the information and guidance to gain the autonomy in making the best decisions in regards to their healthcare journey. Impact Life Group believes that the more active a patient is involved in their own healthcare, the more they will utilize the variety of healthcare services available to them. It is the goal of Impact Life Group for patients to move outside of simple in-office engagement, and for patients/healthcare consumers be active in their healthcare journey.

Going to an annual appointment and getting prescriptions filled is not the end all and be all of good health, though this is the most that many do. In fact, going for a doctor's appointment is only 20% of healthcare. The other 80% takes place outside of the doctor's office, meaning this percentage depends upon the patient's active participation. With "Patient Activation", Impact Life Group aims for patients to be involved in the direction that their health care journey goes, thus enabling them to give themselves the best possible gift: a healthier and happier life.

We want patients/consumers not to think of better health as something to do outside of their lives, but rather an integration into their day-to-day life. Instead of doctor's appointments looming over a patient's head, Impact Life Group wants them to view those appointments as checkpoints on the progress they have made outside of the office and pit stops to gain updated information so that the patient/healthcare consumer may continue on their personal health care journey.

Patient Activation

The perennial challenge for healthcare organizations as they continue to develop and refine programs aimed at improving healthcare quality while reducing costs is engaging patients in these initiatives. Actively engaged patients have been shown to have lower costs and improved outcomes.

Our findings indicate that one in three healthcare consumers are disengaged and report less preventative action, less interest in commitment and accessibility (both financial and educational), and thus are less financially prepared to help themselves. Impact Life Group believes we can change that by engaging the consumers through education of options available to them and advise them on the specific options there are, including care for chronic medical disorders, adverse health behaviors and outcomes, and behavioral health. Not only can Impact Life Group provide this, we believe this meets a long awaited need. Our findings indicate that 53% of people state that they struggle in understanding their health information, and only 44% of consumers stay engaged in their wellness program throughout the year. However, our findings also indicate that 55% of people want help from their healthcare plan in setting personal health goals, and 65% want reminders about critical health actions and annual health exams. This indicates that not only does a gap in information for health between providers and consumers exist, there exists a demand for the information and services that Impact Life Group uses to bridge that gap.

We envision patients becoming more active and gaining a better understanding of what services best suit them, both for their health and financial needs. Our findings indicate that only 38% of consumers have wellness programs that help them manage their costs, and that 81% of healthcare programs do not offer a price comparison tool. With this in mind, Impact Life Group wants to guide the consumer to a program that better helps meet their needs so that they can better spend their energy and funds on working towards a happier and healthier self. Taking into account the consumer's medical history, needs, and risk factors, we will find the options best suited to the them. Impact Life Group also utilizes Population Health Management in customizing the patient's plan by reducing cost and analyzing the patient's environmental needs, thus enhancing the patient's experience. This joint-based approach allows us to provide and introduce new strategies and approaches to engage patients in their healthcare.

Impact Life Group also helps` patients/healthcare consumers balance their policy goals and with understanding the charge amount reality. With this, we aim to ensure that monetary roadblocks no longer hinder the consumer's commitment to better health. We use a combination of HEDIS, the consumer's medical history, and the patient's health goals in composing the best health care plan possible. Impact Life Group also utilizes Meaningful Use standards to exchange information that is not only helpful in aiding the patient with choosing the best plan, but also to create a more streamline track of information between providers so that the patient can get the most effective and efficient services. In addition, we are in the process of employing a portal for patient engagement efforts, as well as developing the use of health IT tools to engage individuals in population health. This includes remote monitoring, a variety of smartphone apps, virtual visits, and many others methods that are in development.

As well as technological advances, also in development are formations of work groups and work streams dedicated to health literacy. We believe that the more educated a consumer is, and the more accessible we make that information, the more committed they will be to achieving health goals as they will find the process easier as a whole. By not only guiding consumer towards this literacy, but by also providing additional literacy, Impact Life Group will aid in building a better bond between the healthcare providers and the consumers, and the consumers will learn to trust themselves more and thus trust their healthcare providers more.

We at Impact Life Group define ourselves as "an organization of partners for patient care services and activities that are between two or more participants involved in a patient's care in order to facilitate the health and wellness delivery of health care services". We believe in a healthy relationship between patients/healthcare consumer and providers, and believe that they can work together to make happier and healthier lives. We know that our method of providing patient autonomy will set the path for a healthier society as a whole, as well as happier individuals. Let's get healthy together with Impact Life Group.
